Excess adiposity (BMI ≥30 kg/m², or ≥27.5 in South Asian populations) with adverse metabolic and mechanical consequences.

Investigations

  • BMI, waist circumference, BP
  • HbA1c, lipids, LFTs (NAFLD), TFT
  • Screen OSA (STOP-BANG), depression

Management

  • Structured lifestyle: 500–750 kcal/day deficit + 150 min/week activity
  • Pharmacotherapy: GLP-1 RA (semaglutide 2.4 mg, liraglutide 3 mg), orlistat
  • Bariatric surgery if BMI ≥40, or ≥35 with comorbidity
  • Post-bariatric lifelong micronutrient supplementation

Clinical pearls

  • Weight regain after stopping GLP-1 therapy is expected — plan long-term strategy
  • Screen for and treat OSA before elective surgery
