Semaglutide

Dose

0.25 mg SC weekly × 4 wk → 0.5 mg → 1.0 mg weekly (up to 2.4 mg for obesity). Oral: 3 → 7 → 14 mg OD fasting.

Use

T2DM with cardiovascular benefit; obesity management.

Monitoring

Weight, HbA1c, GI tolerance, retinopathy screening if rapid HbA1c drop.

Side effects

  • Nausea
  • Constipation/diarrhoea
  • Cholelithiasis
  • Delayed gastric emptying — anaesthetic risk
