Dose

DKA: 0.1 U/kg/h IV. HyperK: 10 U in 50 mL 50% dextrose IV over 15 min. SC sliding scale per local.

  • DKA
  • HHS
  • Hyperkalaemia
  • Hyperglycaemia

Dilution & preparation

50 U in 50 mL NS (1 U/mL) IV infusion

Use

DKA, HHS, hyperkalaemia, perioperative/critical illness hyperglycaemia.

Monitoring

Capillary glucose hourly, K⁺ 2-hourly, ketones in DKA.

Side effects

  • Hypoglycaemia
  • Hypokalaemia
  • Weight gain (chronic)
