Flecainide

Dose

PO 50–150 mg BD; PIP 200–300 mg single dose. IV 2 mg/kg over 10 min.

Indications

  • Paroxysmal AF ('pill in pocket')
  • AVNRT
  • SVT prevention

Contraindications

  • Structural/ischaemic heart disease (CAST)
  • HFrEF
  • Sinus node dysfunction

Use

Rhythm control in structurally normal heart; co-prescribe AV nodal blocker.

Monitoring

ECG (QRS widening >25% → stop), U&E.

Side effects

  • Pro-arrhythmia
  • Visual disturbance
  • Dizziness
