Enoxaparin
Dose
Prophylaxis 40 mg SC OD; treatment 1.5 mg/kg OD (or 1 mg/kg BD)
Dose adjustment
CrCl <30: prophylaxis 20 mg OD; treatment 1 mg/kg OD
Use
VTE prophylaxis/treatment, ACS.
Monitoring
FBC, anti-Xa if obese / renal / pregnant (target 0.5–1.0 IU/mL).
Side effects
- • Bleeding
- • HIT (less than UFH)
- • Hyperkalaemia
