Apixaban
Dose
AF/VTE: 5 mg BD (2.5 mg BD if ≥2 of: age ≥80, weight ≤60 kg, Cr ≥133); VTE first 7 days 10 mg BD then 5 mg BD.
Indications
- • Non-valvular AF (ARISTOTLE)
- • DVT/PE (AMPLIFY)
- • VTE prophylaxis
Dose adjustment
Avoid if CrCl <15
Use
Preferred DOAC in elderly and CKD (lowest bleeding).
Monitoring
Bleeding, renal function.
Side effects
