AF: 5 mg BD (2.5 mg BD if ≥2 of: age ≥80, weight ≤60 kg, creatinine ≥133) VTE: 10 mg BD × 7 d → 5 mg BD

- • Non-valvular AF (stroke prevention)
- • VTE treatment / prevention
Contraindications
- • Active bleeding
- • Mechanical heart valve
- • Severe hepatic disease (Child C)
Use
No INR monitoring; predictable PK.
Monitoring
Renal function, FBC, bleeding signs.
