HFrEF developing last month of pregnancy or first 5 months postpartum, no other cause.
Management
- • Standard HF therapy (NB: avoid ACEi/ARB/MRA in pregnancy — use hydralazine + nitrate)
- • Bromocriptine 2.5 mg BD × 1 wk then OD × 6 wk (BOARD trial)
- • LMWH prophylaxis (thrombosis risk)
- • Contraception advised for future pregnancies
Complications
- • 50% recover; recurrence rate high if EF not fully recovered
