Antibiotics

Lower UTI in women; pyelonephritis; complicated UTI.

Regimens

  • Uncomplicated cystitis (♀)

    Nitrofurantoin 100 mg PO BD × 3 d

    Avoid eGFR <45.

  • Pyelonephritis

    Co-amoxiclav 625 mg PO TDS or ceftriaxone 1 g IV OD × 7–10 d

  • Catheter-associated

    Treat only if symptomatic; remove or change catheter.

Monitoring

  • Symptoms, fever
  • Repeat MSU only if not improving

Common prescribing errors

  • Treating asymptomatic bacteriuria (except pregnancy/pre-urology)
  • Long course nitrofurantoin in elderly
  • Trimethoprim in early pregnancy (folate antagonist)
