Before the round
- • Print patient list with bed, age, diagnosis, day, consultant
- • Check overnight events, observations, fluid balance, urine output
- • Review bloods, microbiology, imaging
- • List jobs already pending
At the bedside
- • Patient identity, NEWS / EWS
- • Subjective: pain, sleep, appetite, bowels, mood
- • Objective: A → E quick exam, lines, drains, wounds
- • Investigations: bloods, micro, imaging summary
- • Active issues + plan (problem-based)
- • Medication review (STOPP/START, anticoagulation, antibiotics)
- • Fluids, VTE prophylaxis, gastric protection
- • Escalation status / ceiling of care
- • Discharge planning + estimated date
After the round
- • Document plan in notes — date, time, signature, bleep
- • Update electronic jobs / handover sheet
- • Order investigations, write prescriptions, send referrals
- • Speak to family if planned
