Recognition

  • Positive deflection at J-point (junction of QRS and ST)
  • Best seen in inferior + lateral leads
  • Bradycardia, muscle tremor artifact, prolonged intervals

Key findings

  • Height correlates with severity of hypothermia (<30 °C)

Clinical importance

  • Predisposes to VF; handle patient gently

Management pearls

  • Passive and active rewarming
  • Avoid rough movement; withhold shock/pacing until >30 °C if possible
