Recognition
- • Positive deflection at J-point (junction of QRS and ST)
- • Best seen in inferior + lateral leads
- • Bradycardia, muscle tremor artifact, prolonged intervals
Key findings
- • Height correlates with severity of hypothermia (<30 °C)
Clinical importance
- • Predisposes to VF; handle patient gently
Management pearls
- • Passive and active rewarming
- • Avoid rough movement; withhold shock/pacing until >30 °C if possible
