Anticoagulants
Warfarin
Dose
Start 5–10 mg, titrate to INR 2–3 (mechanical valve 2.5–3.5)
Use
AF, mechanical valves, VTE, thrombophilia.
Monitoring
INR daily until stable then weekly→monthly.
Side effects

- • Skin necrosis
- • Teratogenic
