Ticagrelor

Dose

Load 180 mg PO, then 90 mg BD (60 mg BD after 12 months).

Mechanism

Direct reversible binding to P2Y12 receptor.

Indications

  • ACS (medical or PCI) with aspirin × 12 months (PLATO)
  • Prior MI 1–3 y (PEGASUS 60 mg BD)

Contraindications

  • Active bleeding
  • Prior ICH
  • Severe hepatic impairment

Dose adjustment

No renal adjustment

Use

First-line P2Y12 in ACS (superior to clopidogrel).

Monitoring

Bleeding, dyspnoea, uric acid, renal function.

Side effects

  • Dyspnoea (14%)
  • Bleeding
  • Bradycardia/pauses
  • ↑uric acid
