Dose
Load 180 mg PO, then 90 mg BD (60 mg BD after 12 months).
Mechanism
Direct reversible binding to P2Y12 receptor.
Indications
- • ACS (medical or PCI) with aspirin × 12 months (PLATO)
- • Prior MI 1–3 y (PEGASUS 60 mg BD)
Contraindications
- • Active bleeding
- • Prior ICH
- • Severe hepatic impairment
Dose adjustment
No renal adjustment
Use
First-line P2Y12 in ACS (superior to clopidogrel).
Monitoring
Bleeding, dyspnoea, uric acid, renal function.
Side effects
- • Dyspnoea (14%)
- • Bleeding
- • Bradycardia/pauses
- • ↑uric acid
