Rivaroxaban
Dose
AF/VTE: 20 mg OD with food; VTE first 3 weeks 15 mg BD then 20 mg OD.
Indications
- • Non-valvular AF stroke prophylaxis
- • DVT/PE treatment (EINSTEIN)
- • Post-hip/knee VTE prophylaxis
- • CAD/PAD 2.5 mg BD + aspirin (COMPASS)
Contraindications
- • Active bleed
- • Mechanical heart valve
- • Moderate/severe hepatic impairment (Child B/C)
- • CrCl <15
- • Pregnancy
Dose adjustment
AF: 15 mg OD if CrCl 15–49
Use
OD DOAC — take with food to ensure absorption.
Monitoring
Renal function annually (6-monthly if CrCl <60), Hb.
Side effects
- • Bleeding
- • Anaemia
- • GI upset
Pregnancy safety
Contraindicated.
