Rivaroxaban

Dose

AF/VTE: 20 mg OD with food; VTE first 3 weeks 15 mg BD then 20 mg OD.

Indications

  • Non-valvular AF stroke prophylaxis
  • DVT/PE treatment (EINSTEIN)
  • Post-hip/knee VTE prophylaxis
  • CAD/PAD 2.5 mg BD + aspirin (COMPASS)

Contraindications

  • Active bleed
  • Mechanical heart valve
  • Moderate/severe hepatic impairment (Child B/C)
  • CrCl <15
  • Pregnancy

Dose adjustment

AF: 15 mg OD if CrCl 15–49

Use

OD DOAC — take with food to ensure absorption.

Monitoring

Renal function annually (6-monthly if CrCl <60), Hb.

Side effects

  • Bleeding
  • Anaemia
  • GI upset

Pregnancy safety

Contraindicated.
