Lamotrigine

Dose

25 mg OD × 2 wk → 50 mg OD × 2 wk → increase 50–100 mg q1–2 wk (usual 100–400 mg/day). Halve dose with valproate.

Use

Focal and generalised epilepsy; bipolar depression maintenance. Preferred antiseizure drug in pregnancy.

Monitoring

Rash (stop immediately), seizure frequency; levels fall in pregnancy — recheck each trimester.

Side effects

  • Rash / Stevens-Johnson syndrome
  • Headache
  • Diplopia
  • Insomnia
