Heparin (UFH)
Dose
VTE: 80 U/kg IV bolus, then 18 U/kg/h infusion, titrate to APTT 1.5–2.5×
Use
ACS, VTE, dialysis circuit, renal impairment alternative to LMWH.
Monitoring
APTT 6-hourly, FBC (HIT after 5–14 days).
