IV 25–100 mcg q1–2h PRN; infusion 25–200 mcg/h; lozenge / patch chronic

Mechanism

Potent µ-receptor agonist (~100× morphine)

Indications

  • Acute severe pain
  • Anaesthesia adjunct
  • Procedural analgesia

Use

Rapid onset (~1 min), short duration; haemodynamically stable.

Monitoring

RR, sedation, SpO₂, pain score.

Side effects

  • Respiratory depression
  • Chest-wall rigidity (large bolus)
  • Bradycardia
  • Pruritus

Pregnancy safety

Use if needed; neonatal respiratory depression if near delivery
