10 mg PO OD (25 mg in T2DM if tolerated)
Mechanism
Blocks proximal tubular glucose reabsorption → glycosuria, natriuresis
Indications
- • T2DM
- • HFrEF / HFpEF
- • CKD with proteinuria
Contraindications
- • Type 1 DM (DKA risk)
- • Volume depletion
Dose adjustment
eGFR ≥20 acceptable for HF / CKD indication
Use
Cardiorenal protection beyond glycaemia.
Monitoring
Renal function, ketones with illness, volume status.
Side effects
- • Genital mycotic infection, UTI
- • Euglycaemic DKA
- • Volume depletion
- • Fournier gangrene (rare)
