Dabigatran
Dose
150 mg BD (110 mg BD if age ≥80, high bleed risk, or verapamil co-therapy).
Indications
- • Non-valvular AF (RE-LY)
- • VTE treatment/prophylaxis
Contraindications
- • Mechanical heart valve
- • CrCl <30
- • Prosthetic valve
Dose adjustment
CrCl 15–30: 110 mg BD; <15 avoid
Use
DOAC reversed by idarucizumab.
Monitoring
Renal function q6–12 months.
Side effects
- • Dyspepsia (10%)
- • Bleeding
- • MI signal (small)
