Bivalirudin
Dose
0.75 mg/kg bolus, then 1.75 mg/kg/h during PCI.
Indications
- • PCI in ACS (particularly if HIT)
- • HIT anticoagulation
Dose adjustment
CrCl <30 or dialysis: 1 mg/kg/h
Use
Alternative anticoagulation for PCI, especially in HIT.
Monitoring
ACT (target 250–300), bleeding.
Side effects
- • Bleeding
- • Stent thrombosis (early)
