Acyclovir
Dose
HSV encephalitis: 10 mg/kg IV q8h × 14–21 days (ideal body weight). Zoster: 800 mg PO 5×/day × 7 days.
Use
HSV encephalitis, disseminated/ophthalmic zoster, severe varicella.
Monitoring
Renal function and hydration (crystal nephropathy), neurotoxicity in CKD.
