Indication
ICU sedation, RSI induction, refractory status epilepticus.
Preparation
Pre-mixed 10 mg/mL emulsion via dedicated line (no dilution required).
Dilution
10 mg/mL (1%) — undiluted
Rate
Sedation 0.3–4 mg/kg/h; induction 1.5–2.5 mg/kg IV bolus.
Monitoring
- • BP (hypotension)
- • Triglycerides if >48 h
- • CK / lactate (PRIS)
- • Sedation depth (RASS)
Common errors
- • Running >4 mg/kg/h for >48 h → propofol infusion syndrome
- • Sedation without analgesia
Pearls
- • Always pair with opioid for analgesia
- • Change tubing q12h (lipid emulsion → microbial growth)
